KDA_3_30511A1 - --In the dry storage area there was one box of pre-package coffee being stored directly on the floor. COS-the PIC removed the box off of the floor.
KDA_4_20211A2 - --On a hook above the 3 compartment sink there was one black plastic tong and one blue plastic tong with melted edges (not easy to clean and can be a physical hazard). COS-discarded.
KDA_4_60111A - --On the rack above the 3 compartment sink there was one metal sheet pan with dried food debris and stored clean. On a hook above the 3 compartment sink there was one red plastic tong with food debris on the food contact surfaces. On the utensil rack to the right of the 3 compartment sink there was one plastic squirt bottle with a time sticker left on it and stored clean (can hold bacteria). There was also one plastic lid, one 1/3 plastic food grade pan and four 1/6 plastic food grade pans with dried food debris/oil residue and stored clean. There was one 5.2 L plastic food grade container with stickers left on it and stored clean. There were three 1/3 plastic food grade pans with dried food debris and stickers left on them and stored clean. COS-moved all of the items to the ware wash area.

KDA_5_20212A - --The men's restroom handsink was reading 96.8F and the women's restroom handsink was reading 95.5F. COS-adjusted the temperature gauge and now they are both reading 100F.
KDA_5_20515B - --The handsink (located by the food prep area) cold water press handle leaks when water is supplied. The valve has been ordered and will arrive on 10/19/15. There are 2 additional handsinks. The left faucet located at the 3 compartment sink leaks when water is supplied. COS-fixed.
