Observation: Master-Bilt cooler gasket, True 3 door cooler gaskets (middle and left), walk in freezer door hinge, walk in freezer (leaking water from roof/condensate line), and walk in freezer walls (peeling paint). All violations will be taken care of by Ace Refrigeration with the exception of the walk in freezer walls. Documentation must be sent to the Health Dept. to verify service from either the facility or Ace.

Observation: Hole in the FRP by the mop sink that exposes the studs and bare wall. Hole must be properly sealed and or capped.

Observation: Several squeeze bottles, shakers, etc. do not have a label.
- Food storage - preventing contamination from the premises
Observation: Food items were stored in the same area as used cans/bottles (far right shed). Cans and bottles were piled up in the entrance of the shed and the rest of the shed was inaccessible.
- Utensils and pressure measuring devices maintained
Observation: Flat top scraper and spatula were both in disrepair.
- Maintaining premises free of litter and unnecessary equipment
Observation: Outdoor shed and fenced in area between the shed and the walk in cooler has what appear to be unnecessary items inside. All items that are not necessary to daily operation must be removed.
- Common name-working containers
Observation: Chemical bottles did not have a label on them.
- In-use utensils, between-use storage
Observation: Cups were being used as scoops in bulk storage containers. If storing a scoop in a product then it must have a handle and the handle must be stored outside of the product.
- Equipment location, installation, repair, and adjustment
Observation: The following equipment is in poor condition and must be repaired or replaced: Master-Bilt cooler (torn gasket), Hussman stand-up cooler @ bar (standing water inside unit), True 3 door cooler on the left (middle and left torn door gaskets), Walk in freezer and cooler (paint chipping of wall/ceiling), Walk in freezer door (hinge broken), Walk in freezer (leaking water possibly from the roof or condensate drain).

Observation: Womens bathroom did not have a hand washing sign posted.

Observation: The following areas have become soiled and must be cleaned: inside of the outdoor walk in cooler (floors, walls, ceilings), behind the pepsi cooler and pop fountain, floor sink @ bar, soap dispenser @ bar, paper towel dispenser @ bar, hand sink @ bar, behind bar equipment, keg fridge (inside, seals, door tracks, etc.).
- Test kit provided and used to measure sanitizing solution concentration
Observation: Did not have test strips for the dish machine at time of inspection.
- Plumbing system maintained in good repair
Observation: The following areas must be kept in good working order: mop sink cold water not working, mop sink faucet leaking hot water, and the condensate drain line in the outside walk in cooler is leaking.

Observation: The following areas do not have smooth, easily cleanable, and non absorbent floors, walls, and or ceilings: both walk in coolers (holes in ceilings), walk in freezer (holes in ceiling), ceiling tiles (poor condition), hole in FRP by the mop sink.

Observation: Black rack in the kitchen has bare wood exposed, and the Black Shelving at the bare is peeling paint.

Observation: The following areas have become soiled and must be cleaned: hood filters, walk in coolers/freezer fan covers, kitchen air vents (ceiling), Hussman stand up cooler has a dusty vent cover.
