Observation: Sides of the counters and on top of the counters were not clean to sight.

Observation: The PIC could not remember the components of the sick policy. I provided her another sick policy and went over the policy with her.

Observation: Raid is unapproved for the food service area. The product was removed.

Observation: The hand sink in the prep area had several utensils being stored in it. The units were removed.

Observation: The facility did not have a diarrhea vomit clean up plan. I provided the PIC with one and she stated she would copy it and place it into protocol.

Observation: The person in charge did not know what temp what temp her fridge unit needs to maintain.
- Food employees hair is effectively restrained
Observation: The person in charge did not have a hair restraint.

Observation: The facility does not have a procedure for cleaning up diarrhea and vomit.

Observation: The three hole sink had several things being stored on top of the sink.

Observation: The person in charge did not know what the 5 food borne illnesses were and the 5 symptoms of food borne illness.

Observation: The prep area was not clean to sight. Clean the area.

Observation: The sink was filled up with water cleaning a dish when I entered the facility.

Observation: The employee was handling cones with her bare hands.
- PHF/TCS foods maintained at 41°F or below, except during preparation, cooling, cooling, or when time is used as a public health control
Observation: The fridge unit was operating at 49-50 degrees F.

Observation: The hand sink did not have hand towels when I arrived.
