    Observation: The ice chute by the drive through was not clean to sight. The unit was being cleaned when I was leaving the inspection.

    Observation: One ice bucket was not labeled. The unit was labeled prior to me leaving.

    Observation: The back door was not sealed to the ground.
  • Non-food contact surfaces and food contact surfaces of cooking equipment and pans are clean
    Observation: A couple of containers that were storing utensils were not clean to sight.
  • Handwashing signage
    Observation: The hand sink in the prep area did not have a hand wash sign placed by it. I provided a hand wash sign and it was placed by the sink.
  • Storage of clean linens, single-service, and single use articles
    Observation: Single use spoons were not stored in one direction behind the cashier.
  • Food employees hair is effectively restrained
    Observation: Several employees were prepping food without hair restraints.
  • Storage of clean equipment and utensils
    Observation: Utensils were being stored with cleaning equipment in one bucket. The units were moved.
  • Storage and maintenance of wet and dry wiping cloths
    Observation: Several wiping cloths were being left on the counter tops after each use.
  • Sanitizers
    Observation: The sanitizer was 0 ppm. The units were remixed and the units were 100 ppm prior to me leaving.

    Observation: The vents in the walk in cooler were not clean to sight.

    Observation: One manager was not wearing gloves while preparing food on site.

    Observation: A manager on site went to prepare a meal on the line and was not wearing gloves. I discussed the importance of wearing gloves to eliminate bare hand contact while preparing sandwiches on the line.

    Observation: There was a window cleaner bottles being stored above a prep area. The bottle was moved prior to me leaving.

    Observation: The American and Swiss cheese time mark was not updated. The employee updated the American and Swiss cheese time marks prior to me leaving.

    Observation: The ice machine that was by the drive through was not protected.

    Observation: A ceiling tile was missing in the prep area.

    Observation: The facility did not have a written plan for cleaning up diarrhea and vomit. I gave them the state diarrhea vomit clean up guidelines. They stated they would create their own from there.

    Observation: The vent screens in the walk in cooler unit were not clean to sight.

    Observation: Several wiping cloths were laying on top of counter tops by the meat prep area and the assembly line area.

    Observation: The sanitizer bucket for common towels was maintaining 0 ppm. The sanitizer bucket was remixed and was 50 ppm.
