- Protecting food from environmental contamination
Observation: raw chicken being stored on kitchen floor near stand up cooler. Owner advised was to be prepared at 1:00 p.m. time was 11:30 a.m., removed to cooler. Flat of eggs sitting on stand near cook line, moved to cooler.
- Food storage - preventing contamination from the premises
Observation: box of crispy noodles in restroom in kitchen area being used as a store room. Asked owner what was being done with the noodles advised boiled and served. Box of noodles discarded during inspection.
- Handwashing signage
Observation: handwashing signs not posted in restrooms. Sign not posted at handwashing sink in kitchen. Posted only after owner was instructed to do so.
- Package integrity
Observation: cans of soy sauce had been removed from shelf from inspection on 5/12/2015.
- Premises maintained free of insects, rodents, and other pests
The presence of insects rodents, and other pests are controlled as required

Observation: kitchen area filthy.

Observation:containers of cornstarch, potato starch, msg, doors on prep table and stand-up cooler not clean to sight or touch. Not cleaned from inspection on 5/12/2015. Owner started cleaning after being instructed to do so during inspection.
- Using a handwashing sink- operation and maintenance
Observation: aprons and wet paper towels laying in hand washing sink in kitchen upon arrival. Removed immediately by owner.
- Certified Food Protection Manager with management and supervisory responsibility employed by the establishment
Observation: no CFPM on staff. Discussed with owner needs to be done within six months.

Observation:items on buffet: noodles 111 degrees - cabbage 105 degrees - sweet & sour chicken 120 degrees - potatoes & vegetables 120 degrees - all removed from service.

Observation: gloves in handwashing sink. Container of steamed rice on cart in front of handwashing sink blocking access. Sink dirty to sight. Soap dispenser dirty. All corrected at time of inspection.
