Observation: Manager could not produce employee health reporting forms as they were locked up in a tote.

Observation: Roast temping at 124-127 degree over hour long period, temped multiple times. Corrected on site by discard.

Observation: Burger make table vegetables and cheeses holding 49-66 degrees. Corrected on site by discard and conversation about setting items lower into table as the containers were sticking up a couple of inches.

Observation:Walk-in refrigerator is in disrepair and has a beam jack and board supporting the ceiling. Floor of same is non compliant and loose. Current owner has provided documentation that unit will be replaced.

Observation:Walk-in refrigerator is running warm. Air coming out of chiller at 49.5 degrees, shredded cheese measures 45.9 degrees, precooked ground beef patty 45.8, cut tomatoes 51.0 degrees. Records indicate refrigerator was 38.0 at 6:30 a.m. All TCS food may be moved to other properly working refrigeration. Manager stated service has been called and is coming.
