Observed 6 employees working on 9/20/13 with no certified food manager present. Katina stated she is currently training to be a CFM but is not yet certified. This violation must be corrected by Wednesday, November 20, 2013. **Warning** Callback 11/27/13: Observed ServSafe certificate for owner Angelo D. Crowell but he was not on staff at time of callback or routine inspection. Observed four employees on staff at time of callback. At callback, Morgan Gilmore (Store manager) said she had just taken and passed exam to be certified food manager but has not received certificate yet. Time extension of 30 days granted. This violation must be corrected by December 27,2013.

Observed 4 employees on staff, employed more than 60 days, with no proof of state approved employee training: Katina, Leah, Matthew, Debony. Katina stated she is about to become a CFM. Katina contacted Owner Angelo Crowell who stated he owns 4 Jersey Mike''s stores in Tallahassee and recently rotated his staff between the stores and that training certificates were at the employees last location. Owner was advised since he owned multiple stores and interchanged his staff, he is allowed to make copies of employees food safety training to keep at any and all locations they may work at that he owns. This violation must be corrected by Wednesday, November 20, 2013. To order approved program food safety material, call DBPR's contracted provider: Florida Restaurant and Lodging Association (SafeStaff) 866-372-7233. **Warning** Callback 11/27/13: Observed no completed State approved employee training for anyone employed at establishment.
